A little preparation helps you get the most out of your first evaluation. Use this checklist in the days before your visit.
Gather this information
- A list of all current medications and supplements, with doses
- Medications you have tried before: what helped, what did not, and any side effects
- Past diagnoses and any hospital stays for mental health
- Family history of mental health conditions, if you know it
- Your therapist’s or primary care provider’s name, if you have one
- Recent lab results, if you have them
- Your insurance card and photo ID
Think about these questions
- What symptoms bother you most, and when did they start?
- How are sleep, appetite, energy, and concentration?
- What makes things better or worse?
- How much alcohol, cannabis, or other substances do you use? (Honest answers help us keep you safe.)
- What would feeling better look like for you?
