Depression test (PHQ-9): free online screening

Medically reviewed by Dr. Iman Yunis, DNP, PMHNP-BC · Last reviewed October 26, 2026

The PHQ-9 is a widely used 9-question depression screening. It takes about 2 minutes, and your answers are not saved or sent anywhere.

The short answer

The PHQ-9 is a 9-question depression screening used by doctors and nurse practitioners worldwide. Scores range from 0 to 27; scores of 5, 10, 15, and 20 suggest mild, moderate, moderately severe, and severe symptoms. It is a screening tool, not a diagnosis, so a high score is a reason to talk with a provider.

If you are in crisis or thinking about harming yourself, call or text 988 or call 911 now. This website is not monitored in real time.

About the PHQ-9

What your score means

Each answer is scored 0 to 3, for a total of 0 to 27. Scores of 5, 10, 15, and 20 are commonly used as cutoffs for mild, moderate, moderately severe, and severe depressive symptoms. A screening is not a diagnosis.
