The short answer

The Adult ADHD Self-Report Scale (ASRS v1.1) is a screening questionnaire developed with the World Health Organization. Answering 4 or more of the first 6 questions in the shaded range suggests symptoms consistent with adult ADHD and a full evaluation is worth considering. It cannot diagnose ADHD on its own.

If you are in crisis or thinking about harming yourself, call or text 988 or call 911 now. This website is not monitored in real time.

About the ASRS

How the screener works

Each question has a threshold. For questions 1 to 3, answers of Sometimes or more count toward the score; for questions 4 to 6, answers of Often or more count. Four or more answers at or above the threshold are highly consistent with adult ADHD and warrant a full evaluation. The screener is not a diagnosis.

Teens 13 to 17 should be evaluated directly, with a parent or guardian involved.
